The phrase began as someone who is pitiful looking. They look like disastrous mess, like they've been to hell and back. Usually they're unkempt and sloppy in appearance.
Then the phrase evolved into applications for situations, not just people. A traffic jam can be a hot mess. A bedroom can be a hot mess. Even a line at the post office can be a hot mess.
It's a term I particularly like to use as I think it's a pretty funny way to describe something that just needs fixing fast. If you ever see anything that's just a hot mess, and you can help make it better, please do so fast!

1. A Hot Mess (noun) - used to describe someone that has no reason to look the way they are looking at the time and also describes someone who is "lookin' like a damn fool"
2. A Hot Mess - A person who is a handful, he/she is a piece of work, and/or a colorful character
3. A Hot Mess - When something just ain't right or is "off da chain"
4. A Hot Mess - Usually the result of a hangover, a Hot Mess can be described as the diarrhea or abnormal bowel movements you have the morning after a night of heavy drinking

Hot mess! Really, if you want to put a contemporary spin on it, it is most often used to describe some one (or some thing) that is shameful, disgracing, or ignorant in appearance or actions.
Urbandictionary.com says the word is:
Someone or Something that is such a mess... the level of it, is off of the charts. It's past pathetic, past pitiful. It's to the point you almost have to walk away to keep from bustin a gut. Hot messes can exist in levels.
But there’s a lot more to it than that. The poster on Urban Dictionary had it right when they said it can exist in levels- withthe signifier going between the "hot" and the "mess." For example, the word really came into popular use with the TV show that is now called We Got to Do Better, which was originally called Hot Ghetto Mess. Wikipedia says:
